Do You Have Questions About Your Child's Development? ​

During the first 5 years of life, your child's brain and body are growing rapidly.
Do you want to learn more about your child's development?

Parents or caregivers of children between the ages of birth and 6 years of age living in the southern fifteen counties in Illinois will soon  have access to a special  online questionnaire that helps them check on their child's general development. The results help determine if a child's development is on schedule.

What do I need to do?

  • Complete the  child and family information form
  • Review and accept the information on the parent consent form.
  • Answer a few questions about your child and family
  • Fill out the age appropriate Ages and Stages Questionnaire for your child

What happens after I complete the questionnaire?

  • Results will be sent to you by email at your request
  • Results will also be sent to your child's teacher at your request.
What If There are Concerns?

A parent, guardian, teacher, or doctor can call the Child and Family Connections in your area for a free child's developmental screening, ages birth to three years, in your home or convenient location:

For children 3 years and above, families and professionals should contact the local school district the child will be attending.

Remember that developmental activities are tips are always available in the parenting section of the sifamilies.org website.

Parents are their child's first and most important teacher. If you have concerns about your child's development you should contact your physician or call 1-800-548-5563 to get information about more resources in southern Illinois.
