What happens after I complete the questionnaire?
- Results will be sent to you by email at your request
- Results will also be sent to your child's teacher at your request.
What If There are Concerns?
A parent, guardian, teacher, or doctor can call the Child and Family Connections in your area for a free child's developmental screening, ages birth to three years, in your home or convenient location:
